Step 1
Complete Patient Form
If you're a new patient: Please print and fill out the "New Patient Form" in its entirety prior to your initial appointment. If you don't have access to a printer, please arrive 5-10 minutes early to your appointment in order to complete it.
If you're a referring dentist: Please print and fill out the "Patient Referral Form" and fax or email it to our office.
